At Chen Family Dentistry of Rochester, PLLC, our goal will always be to treat our patients with the respect they deserve and provide them with a warm and welcoming environment where they will receive the highest quality of dental care available. Appointments
Once you make an appointment at our office, that time will be reserved for you. We send you a courtesy reminder for your appointment via phone call, text or email, whichever you prefer. Please arrive on time. By being prompt, you can take advantage of all the time we have dedicated to you.
If, for some reason, you are unable to keep your scheduled appointment time, please call us with 48 hour notice to reschedule.
Have updates? Please inform us as soon as possible should you have any change regarding your medical condition, dental insurance, or contact information.
Covid-19 Concern:
UPDATE 4/21: For the health and safety of our staff and patients, the entire staff at Chen Family Dentistry of Rochester has been fully vaccinated against COVID-19.
Infection control has always been our top priority. Our office follows infection control recommendations made by ADA (the American Dental Association), CDC (the U.S. Centers for Disease Control and Prevention) and OSHA (the Occupational Safety and Health Administration). We check on updates constantly and adhere to each agency’s recommendations.
You may see some changes when it is time for your next appointment. We made these changes to help protect our patients and our staff.
We will ask you to fill a screening form (see attached) related to Covid-19. Please read carefully. If you have any of the symptoms listed in the form please inform us immediately. We will need to decide whether the treatment should be postponed or completed depending on your answers to the screening questions.

We are GOING GREEN. We encourage patients to fill all the forms digitally! This not only saves time at your appointment, but ensures your information is received as quickly and securely as possible in our practice management software. Please click on the following links and fill them out.
Please read the following by click on the links below. You will be asked to sign those forms when you come into our office.
If you have a printer at home and rather than filling out all the forms digitally, you can click on following tabs, print the forms, fill them out at home and bring them to our office.
Please arrive 15 minutes before your scheduled appointment time to ensure all the required forms are completed before you step into our operatory. This way, we can fully utilize the allotted time for your dental care.

Your First Appointment
To provide you with prompt dental treatment, please be sure to bring the following items when you visit our practice for your initial visit:
- Insurance Information/Card
- Photo Identification
- Any medical/dental records pertaining to your visit including any x-rays, other imaging, labs and studies if applicable
- A list of medications you are presently taking

Insurance Information
We accept most dental insurance plans. Please familiarize yourself with your insurance plan; paying close attention to your benefit maximums, deductibles, and covered percentages.
As a service to our patients our staff will:
- Prepare all the necessary forms for your dental benefits
- Bill your insurance company for you and help you with any claim disputes.
However, we remind you that your specific policy is an agreement between you and your insurance company and it is your responsibility to review and to know your insurance plan.
Payment Information
Payment is expected at the time services are rendered, unless other arrangements have been made prior to the service. If you have dental insurance coverage, this includes any co-payments or deductibles and fees over your yearly maximum–as well as non-covered services.
We accept the following forms of payment: cash, check, Visa, MasterCard & Discover, as well as financing through CareCredit.
